The delivery of safe quality care is a priority for the team at Far North Day Hospital. Our Quality and Safety Committee, Workplace Health and Safety Committee and Medical Advisory Committee ensure we comply with external standards by performing routine audits and assessments on our clinical practices, processes, and policies.

Far North Day Hospital is currently accredited against the National Safety Quality Health Service (NSQHS) Standards, which are established by the Australian Commission on Safety and Quality in Healthcare (ACSQHC).

Far North Day Hospital credentialing is completed via an online system Equifax that allows clinicians to create and maintain their own profiles via a secure platform. The system is easy to navigate and eliminates any manual applications.

New Users (Applicants that do not have an Equifax account)

• Please create a profile via https://myclinicalprofile.com.au/
• Ensure all sections are completed. (Note: Under the Professional Development” section if you do not hold any CPD, please state “Do not hold any CPD” and provide a dummy date i.e. 01 January 2000, to complete your application). 
• Once your profile is completed, please email admin@fndh.com.au with the below details:
• Email addressed used for registration
• APHRA Number
• Clinical Scope of Practice
• You will then receive an email with an invitation link to submit your profile to Far North Day Hospital.

Current Users ( Applicants that have an Equifax account)

• Email admin@fndh.com.au with the below details:
• Email addressed used for registration
• APHRA Number
• Clinical Scope of Practice
• You will then receive an email with an invitation link to submit your profile to Far North Day Hospital.

Please note: As per our Credentialing By-Laws, applications will not be processed unless they include:

• All relevant copies of qualifications and certificates
• Current Identification (i.e. Licence or passport)
• Current Insurance Policy stating:
• Coverage (speciality)
• Policy Claim Limit (i.e. $20 million)
• Period of insurance
